One of the fastest and easiest ways to add image choices is with the WPForms plugin. Here‘s how it works:

Step 1: Install and Activate WPForms

First, install the WPForms plugin from the WordPress repository or your dashboard under Plugins > Add New.

The free WPForms Lite version works great. Once installed, activate it.

Step 2: Create a New Form

From your WPForms menu, click "Add New" to build a new form. Give it a name at the top.
